Bone Grafts Q & A

There are many types of bone grafting material currently available and research promises more to come. Commonly used types include: Allografts (human bone), Bio-oss, Emdogain, and Gem-21. Several types of synthetic products are also available. Recently, a product utilizing recombinant gene technology derived protein (bone morphogenic protein) has been introduced to the dental market with FDA approval for use in sinus grafting and in certain types of ridge augmentation. Allograft

An allograft is a human cadaver bone. Demineralized freeze-dried bone allografts can be effectively used to regenerate bone which has been lost due to the progression of periodontal disease. Bone allografts are commercially available from tissue banks and contain a bone-inductive protein named BMP-3 which has been shown to enhance osseous (bone) regeneration.

 

Emdogain

New regeneration results in pocket depth reduction and the reduction of aggressive bacteria that is found in deep pockets. Patients with good oral hygiene can more predictably disrupt the organization of sub-gingival bacterial plaque. Shallow pockets are also easier for the dental professional to maintain.

Emdogain is composed of enamel matrix proteins. During regenerative surgery, the gel is applied to root surfaces and the surrounding bone. Similar to the way stem cells work to generate new cell growth, Emdogain uses proteins, which your body produces naturally, to regenerate lost gum tissue, bone and the structures that anchor teeth to bone. With more than 400 clinical studies demonstrating its effectiveness, Emdogain is designed to reverse the effects of gum disease and saves teeth.

PuraXen is derived from cow bone that has been processed to remove protein and acts as a stimulus for the body to replace it with new bone.

GEM 21S (growth-factor enhanced matrix)

Gem 21S is a state of the art treatment for periodontal defects. As periodontal disease progresses without treatment, the loss of gum tissue, bone, and other supporting structures becomes more severe. If left untreated, you may experience pain, swelling, loosening of teeth and eventually tooth loss. Gem 21S is implanted under your gums into areas of bone loss. Your natural healing process is sped up by Gem 21S acting like a magnet, attracting specific cells that your body needs to regenerate bone and the surrounding tissues, causing them to multiply in number. This increase in cellular activity results in a more rapid healing process.

Treatment with Gem 21S provides both a platform for regeneration to occur and an active growth factor in which stimulates regeneration of bone and tissue. The growth factor in Gem 21S is called platelet-derived growth factor (PDGF.) PDGF is a wound-healing agent normally found in blood platelets. Platelets are cells normally involved in tissue repair. PDGF is released from platelets at a site of injury and stimulates the initiation of healing. Gem 21S captures the power of this well-known agent by delivering PDGF in a highly concentrated purified form, directly to the treatment site.
